Athletic Forms

All students entering 7th, 9th, and 11th grades for the upcoming 2009/2010 school year who plan on being in athletics are required to get physicals BEFORE they compete in an athletic competition for the 2009-2010 school year. The form you will use is the Preparticipation Physical Evaluation Form. You must have this form with you in order to have a valid physical per UIL rules.
Also:
Every student athlete 7th-12th grade will have to complete the following two forms every year. The two forms are the Illegal Steriod Use & Random Steroid Testing and the Rules of Acknowledgement Form. These forms need to be completed before any athletic competition.
